Community Care Program


If you are under 65, or under 50 for Aboriginal and Torres Strait Islander people and need help to stay living at home and in your community, you may be eligible for assistance from Community Care.

This program provides basic support (usually one to five hours per week) to people with a disability or condition that restricts their day-to-day living.



Anglicare SQ provides care and support services including end of life care.

The Help at Home service includes home care, home nursing, home modifications and access to transport.

Cittamani Hospice Service


The staff and volunteers of Cittamani support people at the end of life.
The care offered is tailored to each client’s needs and is provided free of charge:

In-home respite
Bereavement support
Spiritual support

Metro North Palliative Care Service (Redcliffe)


Metro North Palliative Care Service provides inpatient and outpatient care to clients in the northern suburbs of Brisbane.

The Redcliffe team provides service north to Kilcoy. Care is provided at Redcliffe Hospital with regular clinics at Caboolture and Kilcoy.



Karuna offers a unique in-home and community based palliative care service to people of all ages from little children to our most senior citizens. Karuna’s Home & Community Palliative Care is a no-fee service accessible throughout Brisbane’s Northern suburbs, Caboolture and Redcliffe.

Little Haven Palliative Care


Little Haven Palliative Care is a not for profit, public benevolent institution providing compassionate and professional palliative care and cancer support to people with an incurable illness and their families and friends in the Gympie and surrounding area. Little Haven’s Home & Community Palliative Care is a no-fee service accessible throughout the Gympie and Cooloola Region.

Bloomhill Cancer Help


Bloomhill offers immediate emotional support for any person going through the crisis of being diagnosed with cancer. This help extends to include family members and their carers from time of diagnosis.

Cairns Integrated Palliative Care Service


Inpatient symptom management, respite and end of life care in 12 bed unit Gordonvale Hospital, Gordonvale.

Outpatient and inpatient specialist physician consultation, Cairns Hospital.

Community Palliative Care Service based at Gordonvale Hospital provides in home specialist nursing, medical and social work support. Includes equipment, in home respite, domestic support, 24 hour phone support.

Townsville Palliative Care Service


Townsville Palliative Care Service provides in-patient care for complex symptoms and end-of-life care. Provides consultation to Townsville Hospital, outreach medical, social work and allied health services.

Metro North Palliative Care Service (Chermside)


Metro North Palliative Care Service (Chermside) provides palliative care services in the northern and western suburbs of Brisbane.

Inpatient care is provided at The Prince Charles Hospital and Holy Spirit Hospital, with outpatient clinics at Chermside, and outreach consultancy to clients at home and in residential aged care facilities.



LifeTech aim to connect people and communities through assistive technology. Health professionals available for assessment, education and consultation. Business hours from Monday to Friday, 8.30am-4.30pm, Sat 9am-2.00pm.

National Continence Helpline


National Continence Helpline is a free telephone advisory service funded by the Australian Government Department of Health.

The Helpline is staffed by nurses providing information, education and advice to callers with incontinence, carers and health professionals.

Business hours are from 8.00am - 8.00pm Monday-Friday. (AEST) Other languages and National Relay Service are available through this service.
