Alzheimers Queensland (AQ)


Alzheimers Queensland provides statewide support, education, information, and direct care services. The Association owns and operates three Aged Care Homes in Brisbane, four Multi Service Centres in Toowoomba, Brisbane and Ipswich and a 24 hour Advice Line.

The Multi Service Centres offer but are not limited to: home based and emergency respite, carer support group meetings, activity programs and young onset dementia services.

Community Care Program


If you are under 65, or under 50 for Aboriginal and Torres Strait Islander people and need help to stay living at home and in your community, you may be eligible for assistance from Community Care.

This program provides basic support (usually one to five hours per week) to people with a disability or condition that restricts their day-to-day living.



Anglicare SQ provides care and support services including end of life care.

The Help at Home service includes home care, home nursing, home modifications and access to transport.

Blue Care Queensland


Blue Care provides aged care living, help at home and respite.

Blue Care can provide palliative care services to community clients and residents of aged care facilities including bereavement support.

Ozcare is a statewide service that can work with your GP or Specialist Palliative Care Service to provide palliative care for Queenslanders over the age of 18 .

Services include palliative nursing at home or at an aged care facility. Can also provide respite care at home, in a day respite centre or in an aged care facility. Other home services include personal care, domestic assistance and specialist dementia services.

Mackay Hospital and Health Service Palliative Care


Mackay Hospital and Health Services Palliative Care offers care and support in Emergency, In-home and Hospital settings along with Palliative Care information and education for patient, carer and family members.

A referral from a GP or other doctor is needed to access the service.

In an emergency the patient can attend the hospital Emergency Department to receive appropriate care and referral.

CURA Community Services


CÜRA Community Services is a provider of Home Care Packages and the Commonwealth Home Support Program through the Federal Department of Social Services and by the Queensland Government (Department of Communities, Child Safety and Disability Services.)

Home Instead Senior Care


Home Instead Senior Care is a provider of in-home care for older people that operates in various locations across Queensland.

Services include but are not limited to: Assistance with personal care, light household duties, meal preparation, medication reminders, respite for caregivers, transport to appointments, shopping and social outings.

The Wesley Palliative Care Unit


The Wesley’s palliative care unit has 17 beds, and provides a multidisciplinary service, with medical specialists and registrars, experienced palliative nursing staff, dieticians, physiotherapists, occupational therapists, nurse counsellors and chaplains.

Metro North Palliative Care Service (Redcliffe)


Metro North Palliative Care Service provides inpatient and outpatient care to clients in the northern suburbs of Brisbane.

The Redcliffe team provides service north to Kilcoy. Care is provided at Redcliffe Hospital with regular clinics at Caboolture and Kilcoy.

Wynnum Hospital


Wynnum Health Service is a 21 bed, subacute facility located in Whites Road, Lota. It has 15 general medical and rehabilitation beds in addition to six dedicated palliative care beds.

Dove Cottage – Caloundra Hospital


Dove Cottage is a purpose built 10 bed in-patient Palliative Care Unit on the Caloundra Hospital campus.

Sarina Hospital is a 16-bed hospital that provides emergency care, outpatients, general sub-acute admissions, palliative care, and voluntary elective alcohol detoxification to the Sarina township and surrounding communities.

Toowoomba Hospital Palliative Care


Palliative care at Toowoomba Hospital is provided in a 24 bed unit. There are also community outreach services available.

Other support services available include social workers, physio, dietitian, occupational therapist and equipment for hire.

A referral from a doctor is necessary to access this service.

Toowoomba Hospice


Toowoomba Hospice Association is a community based private healthcare facility which provides free palliative care to the terminally ill.
The Toowoomba Hospice facility has been designed to provide patients with a family friendly "home like" environment during the final phase of their life.

The Toowoomba Hospice nursing staff are dedicated to providing professional palliative care, in a loving, caring and supportive environment, to clients and their families.
​Hospice care is provided to anyone with a life-limiting health condition, who their doctor believes to be in the end-stage of their illness, needing a level of care that cannot be safely provided at home, or if the client does not wish to die at home.

​​Clients may be referred to the Hospice by themselves, their families and friends, their GP, Community Service Providers and other hospitals. For more information about referring a client please contact our Director of Nursing.

Hopewell Hospice Services


Hopewell Hospice offers "Residential and Respite hospitality in a peaceful home-style environment for those who need compassionate palliative care and holistic symptom management."

Hopewell Hospice is a charitable, non-denominational, dedicated palliative care facility servicing the Gold Coast and surrounding regions

Hopewell Hospice is for people with life-limiting illnesses who require permanent accommodation at the end stage of life or shorter-term respite care.

The hospice may be able to support you and your family members by visiting you in your home. Through our outreach service we can arrange for a nurse to visit you if you think that this might suit your needs better.

Referrals can be made directly to the Hospice - by you, your carer or family, your doctor, specialist or other health professionals.

Innisfail Hospital


Innisfail Hospital is a QLD Health 49 bed facility that offers palliative care services.
They also offer allied health facilities including physiotherapy, occupational therapy, social work, radiography, pharmacy and speech pathology.
